MNA® Forms

For nearly 30 years, Nestlé has provided the MNA® forms to users with the object of promoting widespread nutrition screening in the elderly.  Our goal is to identify and treat malnutrition.  The MNA® is available as the “MNA®” or the “full MNA®”.  Many versions are in interactive format.

• The “MNA®” refers to the MNA®-SF, the recommended version of the MNA® for clinical use.
• The ‘full MNA®” refers to the original 18-item MNA®.
• The  “Self- MNA®” refers to the self-administered version

FOR HEALTH CARE PROFESSIONALS:

Clinicians

Clinicians may continue to download the MNA® forms and use them in their clinical practice.  For information on conditions for downloading and using the form go the bottom of the page*.

Healthcare organizations, academic users, universities, scientific societies

These groups must sign a licensing agreement to use the MNA® form.  For information on, or permission to use the MNA® form, please contact Mapi Research Trust** (more information can be found at the bottom of the page).

Commercial users (industry, CRO, any for-profit company)

FOR SELF COMPLETION BY OLDER ADULTS OR CAREGIVERS

The Self-MNA^® Mini Nutritional Assessment is a simple tool that can be used by adults 65 years of age and older or their caregivers. This new tool has been scientifically validated and is as effective as the MNA^® in identifying malnutrition. The Self-MNA^® is available in English with Imperial or US customary measurements, English with metric measurements, Bulgarian, Danish, Dutch, Finnish, French, French(Belgium),German, Greek, Hebrew, Italian, Korean, Lithuanian, Portuguese, Spanish, Swedish and Turkish.

Older adults may click on the desired version to download and complete the Self-MNA^®. Results should be shared and discussed with a health care provider to help optimize nutrition and health.
